Authority to Sign Tax Forms
As detailed in the documents below, Dr. Stucky, Board Treasurer, has authorized the listed individuals to sign the designated forms, electronically or in paper format, on his behalf for the Carbondale and Edwardsville Campuses respectively.
Original copies of the forms must be maintained by the designee’s department until disposed of in compliance with the State Records Management Act.
The authorization and delegation for each individual shall remain in effect throughout his or her continuing service while in the position provided in the document below unless otherwise revoked or modified in writing.
