SIP General and Professional Liability Insurance
The Board of Trustees of Southern Illinois University established the University Risk Management and Self-Insurance Program on July 11, 1985. The department name changed to Risk Management Insurance Programs on August 11, 2024. The program covers legal liability for bodily injury, personal injury and damage to property resulting from occurrences in the conduct of University business, and/or damage arising from professional services. Please note, other parties cannot be added as additional insured.

Requesting a Professional Verification Letter
Please note that a COI is different from a professional verification letter. A COI may be requested by your employer to verify insurance coverage during your employment with SIU SOM.
A professional verification letter will include information for your entire work history with SIU SOM. The letter lists all dates of employment, your medical professional coverage(s) and any claims history you may have. This letter can be sent to you directly, or directly to the hospital/healthcare facility. Customarily, the hospital/healthcare facility will request their own copy through our office.To request a professional verification letter, please complete and sign the request form PDF which includes the Standard Authorization, Attestation and Release form. The release is needed for our office to provide any claims history to a third party with your permission.
